* I switched my fatigue to 10 so I could get a rotation of QBs.
* Misc setting for "Pull Starters" was after 3rd Quarter (my fault)

Why did my starting QB play all game?

If you have 'same for all position groups' your QB is excluded from that calculation - you need to explicitly set the fatigue setting by position groups to get your QB to rotate out.

Also, in order for starters to get pulled, there must be enough depth - in order to sit your starting QB you must have 2 QBs behind him. That way if the #2 QB gets hurt there is still another QB to come in for him.
