So I have been reading a few of the guides and they always refer to the plays by #s and the formations by #s. Where do you actually find these #s that relate to the plays/formations?

For example here he mentions offensive sets 005, 014 and 104.

Those numbers refer to RB, TE, and WR.
005 is a set with five WR.
014 is a set with no RB, one TE, and four WR.
104 is a set with one RB, no TE, and four WR.

Use the "personnel" dropdown on the Scout Offense and Offensive Plays section of the game planning tab.

Since the three numbers always add up to five, some commentators use the NFL two-digit shorthand, which leaves off the wide receivers' number:

00 = 5 WR
01 = 1 TE, 4 WR
10 = 1 RB, 4 WR
11 = 1 RB, 1, TE, 3 WR
etc.

The first number is always RB. The second number is always TE. The (optional) third number is always WR.
