If a play designated as a pass, turns into a run, or more commonly vice versa - does any original key take effect?
eg - Play call is a Run - the Defence has a rule that keys this particular setup as a run - but during the play the QB checks down to an open receiver. Does this suffer any 'penalty' for being keyed as a called run, or is it effectively voided/taken effect when the play breaks down, or does it actually suffer as it is now a run key vs a pass?

In what you desribe i see that as an audible or a run pass option. Both of which are not in this game, yet. But i could see one mixing up a play action pass or a draw/delay as something you described as they are meant fool the defense by design but they still are just a run or a pass in the end.
Once a key is made here it pertains to the offensive call. in the log it will say whether its a run or a pass and tell you whether the defense keyed run or pass in the verbage...,..
once a team, overuses a play or scouts it correctly the AI with start to auto key that play correctly.... stimulating the opponent figuring it out or catching on
