It looks like your kicker is not in the depth chart for field goal kicker, you might want to check that. For the FG distance, that value includes the 10 yards of the end zone (the goal posts are at the back of the end zone) so while he kicked from the 35 yard line it was indeed a 45 yard field goal.

Yes - if you miss a field goal, the other team gets the ball at the spot of the kick, unless it was inside the 20 yard line, in which case they get it at the 20.

If the ball is at the 35, it should actually be a 52 yard attempt. 10 yards for the end zone and generally 7 yards back from the center.

the kick was from the 35 not the line of scrimmage
