It can be done during the offseason stages up through the training camp stage. You must have 6 credits in your account in order to perform a customization, the costs are:
Change name: 3 credits
Change city: 7 credits
Change stadium: 3 credits
Change colors: 5 credits
Change image: 4 credits (if you change the name, city, or colors you can opt to have a banner with the new info included, this is if you want to upload a custom image)
Unlock country: 1 credit (i.e. so you can choose a city outside of the US; this unlocks the country for the entire league so is not necessary if someone else has unlocked the country you want)
Once you make a customization that team will retain it until another one is made in a future season, but if you want to change it again in a future season you'll need to use the credits then too.
