Its a direct snap to the QB, who then tries to hand off to the HB who for some reason doesn't take it. Blown play leads to QB taking off on a run, then shovel passes just before the LOS.

If that was the case, you'd expect to see an illegal blocking downfield penalty, since the rest of the offense wouldn't have realized it was a blown play.
