Here is a group of similar examples from the same game.

NFL League, 2045 season, Conference Championship game, Jets vs. Jaguars.

The Jets have an offensive gameplan rule called "THIRD DOWN (and 6 or 7 yards to go)" that is set up to activate in every possible game circumstance as follows:

Down: Third
Distance: 6 - 7 To Go
Location: Own 0 - Own 25, Own 25 - 50, 50 - Opp 25, Opp 25 - Opp 0
Quarter: First, Second, Third, Fourth, Overtime
Time: 15:00 - 0:00
Score: Down 29+ - Up 29+

This gameplan rule did not "fire" four different times when it should have even though all gameplan conditions were met. We know there was a failure because the play-by-play text does not show the gameplan rule. In the occurrences shown below, all conditions of the gameplan rule are met but no gameplan rule is shown and a random offensive play was selected that is not a play from the gameplan rule had it "fired" properly:

1st Quarter
3-6-NYJ 24 (8:08)
3-Mitchell Christenson pass incomplete, intended for 13-Sarto Veloce . NYJ 64-Bon Fire was injured on the play. He looks like he should be able to return.
Offensive Play: Singleback Normal WR Quick In
Defensive Play: Nickel Normal CB3 MLB Blitz
Pass Key

3rd Quarter
3-6-NYJ 29 (13:40)
4-Van Tovenaar ran to NYJ 26 for -4 yards. Tackle by 48-Kenneth Rowe .
Offensive Play: Shotgun 2 RB 3 WR Shotgun Sweep
Defensive Play: Nickel Normal CB3 MLB Blitz

For the gameplan rule that didn't fire but should have in all four of these 3rd down and 6 yards to go scenarios, I had only medium passes as potential play selections. The rule didn't fire and no medium passes were selected.

Which is almost certainly unrelated to the time issue.

Given downs are really 3/5.957, it's more likely that would be down to a mismatch between displayed data not full reflecting the reality, That's something that would affect narrower distance rules. Not sure if a fix has gone in for that or not.

Did the rule fire at all over the season?

Here's an example of when the rule in question did "fire" as it should.

NFL League, 2045 season, Game #13, Jets vs Browns

4th Quarter
3-6-NYJ 18 (3:12)
3-Mitchell Christenson pass INTERCEPTED by 45-Ray Baynes at NYJ 30. 45-Ray Baynes to NYJ 30 for -0 yards. Tackle by 7-Gregory Spain .
Offensive Play: Singleback Normal HB Release Mid
Rule: THIRD DOWN (and 6 or 7 yards to go)
Defensive Play: Nickel Normal CB3 MLB Blitz

Intercepted too ;)

I suspect that's down to the rounding issue - that the actual distance to go was really outside the boundaries.
